Riverstone Homehealth employs a staff of dedicated caregivers that is thoroughly screened and credentialed, a process that includes a completed application, phone evaluation, personal interview, professional and peer reference verification, extensive background investigation, skill demonstration, initial and annual competency exams, and an evaluation by a Director of Clinical Services (DOCs). Riverstone Homehealth employees have great experience providing in-home care services for adult, senior, and pediatric patients with a variety of conditions.


Riverstone Homehealth ensures that all in-home services are provided for the entire time they are needed. We require that a care representative who is familiar with your specific needs is available 24-hours a day, seven days a week, so high-quality assistance is always at your fingertips. All offices providing skilled care services are licensed by WA State Department of Health, and are recognized as a leading provider of top quality care.


Riverstone and its team of in-home service caregivers specialize in the following treatment areas, and offer the following in-home care services:


Home Health Care- Riverstone homehealth has the experience providing long-term, continuous care to chronically ill patients. Riverstone homehealth team, which provides personal care, management and/or treatment of a variety of conditions includes skilled nurses, therapists, medical social workers (MSWs), and home health aides (HHAs).


Pediatric Care- With Riverstone Homehealth, children receive the necessary assistance they need at home to have a safe, healthy, and happy childhood. Whether a child needs basic nursing assistance or high-tech therapy, the child's family can rely on Riverstone Home Healthcare to provide a healthcare professional that is qualified to perform these vital pediatric services.

Riverstone Homehealt In-Home Services specialize in healthcare for individuals in hospitals and medical facilities, long-term care facilities, assisted living facilities, and private residences. Therapy Services

At Riverstone Homehealth Services, we customize a rehabilitation program around the individual needs of each patient and provide evaluations, treatment, and education for individuals with total joint replacements, orthopedic conditions, neurological disorders, post-surgical procedures, stroke rehabilitation, and speech and swallowing difficulties. Riverstone’s experienced team of therapists offers the necessary assistance to their patients to maintain a safe and independent lifestyle. Rehabilitation>

Riverstone's Rehabilitation Therapists provide treatment and instruction on home safety, balance, orthopedic and neurological rehabilitation, with a focus on returning to independence. Our therapists develop specific home rehabilitation programs to meet the needs of each individual client.

Physical Therapy (PT)

Physical therapy uses physical activity, exercise, and additional treatments such as massage to aid in strengthening, healing, and relieving pain after an injury, illness, or operation.

Riverstone's Physical Therapists assess and provide treatment to help patients of all ages increase mobility, strength, balance, and coordination. Therapists also work with family and community resources to maximize the patient's rehabilitation process.

Occupational Therapy (OT)

Occupational therapy is a rehabilitative service for people with mental, physical, developmental, or emotional impairments that restores or maintains their ability to perform tasks used in daily living, often through developing ways to modify or adapt activities.

Riverstone's Occupational Therapists assess and treat patients in order to increase their self-help and homemaking skills.

Speech-Language Therapy (SLT)

Speech-language therapy involves corrective treatment of physical and/or cognitive disorders that cause difficulty with communication and/or swallowing. Riverstone’s Speech-Language Pathologists help patients develop their language and speech skills to regain control and increase their communication. They also teach patients and family members safe and appropriate swallowing techniques.

Riverstone’s therapists provide one-on-one therapeutic training and support to individuals with physical needs resulting from injury, illness, or surgery in the comfort of their home. Personal Care

Intermittent care enables the patient to receive the highest quality healthcare only when assistance is needed, eliminating the cost of full-time, around-the-clock care. Whether you are recovering from surgery, illness, or managing a disease, we are here to provide nursing care, rehabilitation, and support services in the comfort of your home.

Riverstone Homehealth Services’ Intermittent Care Programs & Resources include:

Skilled Nurses

Regularly scheduled visits by a Registered Nurse (RN) or Licensed Practical or Vocational Nurse (LPN/LVN), ensuring each patient receives appropriate and professional care while attending to all clinical needs.

Certified Home Health Aides

Certified Home Health Aides (CHHAs) provide assistance with activities of daily living and personal care under the supervision of an RN.

Home Care Management

In collaboration with the patient’s Physician, Riverstone's clinical team provides services for individuals with acute or chronic illness. We work with patients to provide individual care management plans that meet their needs. Our Home Care Management Program is accompanied by clinical pathways and disease management programs.

Wound and Ostomy Care

Riverstone Homehealth provides care for surgical and/or pressure wounds, patient education on self care, and instruction for new colostomy patients.

Disease Management

Coordination of care and education in order to provide more efficient and effective care that reduces the short- and long-term complications of conditions such as heart disease, stroke, cancer, and diabetes.

Mental Health

Riverstone Homehealth's Mental Health Program offers services for adults with mental illness, Alzheimer’s disease, dementia, substance abuse issues, or co-occurring disorders. Our Medical Social Workers spend time with each patient in order to develop plan of care that is tailored to his or her personal goals.

Therapy Services

Our therapists customize a rehabilitation program around the individual needs of each patient. Our staff includes Physical Therapists, Occupational Therapists, Speech Language Pathologists, and Medical Social Workers. Riverstone's Intermittent Care Team

Many times, a caregiver may need to leave their loved one at home, but feel uncomfortable leaving them alone without a certified medical caregiver. Our knowledgeable staff conducts in-depth interviews with the patient and family to determine the level of care needed, enabling the patient to receive the care they need from the comfort of their home.

All members of Riverstone’s intermittent care team are licensed. We understand that each individual’s healthcare requirements are unique. That is why we assign only the most qualified and compatible caregivers to meet your specialized needs. Intermittent staff is available on a temporary or long-term basis, depending upon the patient and caregiver need. To begin your care regimen, we will send a nurse or therapist to the home for an initial consultation and collaboration with you and your Physician to develop a detailed and effective plan of care.

Riverstone Homehealth Services, a home health organization that provides continuous and hourly care, is proud to offer a dedicated division that specializes in skilled and unskilled visits in the home. Riverstone Home Health Resources is able to meet all of your intermittent needs by providing service to Medicare, Managed Care, Workers’ Compensation, Private Insurance, and Private Pay clients in the state of Ohio.

Care for People with Disabilities

Riverstone Homehealth Services provide one-on-one staff support for people with disabilities in their homes, offering the necessary assistance to maintain a safe and independent lifestyle. People with developmental and physical disabilities can experience problems with major life activities such as mobility, self-help, and independent living. With the support of a Riverstone caregiver, individuals can participate in activities or programs that might otherwise be inaccessible to them. Pediatric Care

Riverstone’s Registered Nurses, Licensed Practical or Vocational Nurses, Nursing Assistants, and Companions have extensive pediatric experience and deliver care services that are both individualized and highly compassionate towards the needs of the patient and the patient’s family.

Pediatric Care - riverstone Homehealth's pediatric services are accessible around-the-clock or on a per visit, hourly, or daily basis. All Riverstone offices are accredited by the Accreditation Commission for Health Care (ACHC).

Respite Care - Riverstone Homehealth's respite care services offer families the opportunity to take a break from their caregiving responsibilities, leaving their family member or loved one in the hands of a trusted Riverstone Homehealth respite staff member.

Behavioral Health - Raising a child with emotional, behavioral, learning, or developmental issues can sometimes require extra support and guidance. Riverstone Homehealth can help with a behavioral assistance program designed to alleviate inconsistencies in care environment and maintain the long-term objectives of a child's behavioral program.

As an experienced provider of behavioral health programs, our services are designed to meet the unique needs of children and their families, helping everyone overcome obstacles and create foundations for future success. In the comfort of a child's home, Riverstone comes to the aid of children facing ADHD, Conduct Disorders, Oppositional Defiance, Mood Disorders, and other behavioral health concerns.

Riverstone’s behavioral health services include, but are not limited to the following:

Behavioral Health Assistance

Once children make advancements with their treatment centers, schools, or outpatient facilities, they often face the problem of losing progress due to inefficiencies in their new environment. Our behavioral assistance program is designed to alleviate these inconsistencies and maintain the long-term objectives of a child's behavioral program.

We make a strong effort to diminish any regression and assist with emphasizing an interdisciplinary approach to treatment.

All Behavioral Health Assistants must be over age 21 and have a high school diploma/GED, as well as a minimum of one year experience working with children in a comparable environment. Behavior Assistants must attend mandatory one-day training, and complete HIPAA, OSHA, Child Abuse, Domestic Violence, and First Aid training. All Riverstone employees must pass criminal and DMV checks and have a recent TB test.

Intensive In-Community Care

Our in-home therapy services are designed to improve a child and family's capacity to be successful in various life domains. This includes intensive behavior assistance from a Bachelor's level worker, as well as family and individual counseling by a Master or Clinical level therapist.

Our Bachelor level social workers are clinically appropriate to provide these services and must have a Bachelor's degree in a related field and a minimum of one year of experience working with children in a comparable environment.

Our Master level clinicians are required to have a Masters degree in a related field, while our clinical level therapist must possess a LCSW or LPC licensure. Both must also have over one year of experience working with children.

All intensive level workers also attend a mandatory training or orientation as well as complete HIPAA, OSHA, Child Abuse, Domestic Violence, and First Aid training. All Riverstone employees must pass criminal and DMV checks and have a recent TB test.

Mentoring Program

Activities in the home or community often promote independence and personal growth. Our goal is to assist service recipients with meeting their goals and aspirations by introducing a positive role model.

These activities include:

• Assisting a child with homework

• Recreational activities

• Sports

• Sightseeing

• And more

Our mentors demonstrate a commitment to friendship and show respect and sensitivity to each child's individuality and preferences, as well as culture and ethnic diversities.

All mentors must be over age 18 and have at least a high school diploma/GED in a related field. Our mentors will receive supervision by a Program Supervisor and prior to working, will attend a mandatory orientation and complete HIPAA, OSHA, Child Abuse, Domestic Violence, and First Aid training. All Riverstone employees must pass criminal and DMV checks and have a recent TB test.

A Community Approach

Riverstone Homehealth is proud to work with all partners in the system of care. Together, we will help children and their families lead happy and healthy lives.

Our services are available to all children with behavioral needs that are enrolled in their state's Medicaid or otherwise funded programs. Families of children who do not qualify for these programs are still encouraged to contact Riverstone and discuss treatment options.
